2008-2009 Independent Study High School Bulletin
A listing of all courses available from the Independent Study High School for the academic year beginning August 1, 2008.

Enrollment Form. Use this form to enroll in courses.

Change of Status. Fill out a Change of Status form if you need to update your mailing address or your proctor's name or address.

Transcript Evaluation Request Form Submit the Transcript Evaluation Request Form along with your official transcripts from all previous high schools, to receive an advisor's evaluation of your transferrable credit hours.

Transcript Request. Submit the Transcript Request form to have an official transcript from the University of Nebraska-Lincoln Independent Study High School generated for you.

Local School Approval. Use this sheet to gain school approval if you are planning to transfer UNL Independent Study High School credits to your local school.

Lesson Wrapper. Use lesson wrappers when you need to mail in an assignment.

Personal Data Sheets. Please take a few minutes to complete a personal data sheet. This form allows the UNL Independent Study High School staff to get to know you a little better.

Broady Scholarship Application. The K.O. Broady Scholarship Fund provides financial assistance to students in the UNL Independent Study High School. The application, transcript and three references must be on file prior to November 1 for a student to be considered.
